SOAP Based Records

Infinity provides the capability to easily enter medical records in a SOAP based format. By utilizing a unique 4 tab approach, Infinity makes it easy to separate the Notes and Images into their appropriate sections.

S=Subjective:	(Opinion) Reason for patient visit; description of symptoms the client observes in the patient
O=Objective:	(Fact) Findings of a physical exam, results of diagnostic tests
A=Assessment:	(Conclusions) Definitive and differential diagnostics; prognosis
P=Plan:	(Actions) Plan of action for immediate problem (diagnostic tests like lab, radiographic, behavior changes at home with feeding schedules, type of food, etc.). Recommendations for care at home by owner (cleaning ears, how to use meds, brush teeth), post-surgical instructions, referral recommendations, Follow-up directions (re-checks, etc) and what to consider for further treatment/testing if current method does not solve the problem.

Additionally, the SOAP can be used to categorize the medical care provided to the patient. Easily associate any services or invoices to the SOAP and view those records together. This consolidated view of medical records makes it easy to see what treatments were provided for what reason.
